TRANSCRIPT REQUESTS
Upon graduation, each student is provided with two certified copies of their final transcript. Ontario Student Transcripts are also available to all former and current students upon request. If you require an official or electronic copy of your Ontario Student Transcript from Carleton Place High School, please contact Amy Shaw in the Guidance office by email to [email protected] or by phone at 613-257-2720, press 3. You should be prepared with the following information:
1. Your complete first and last name while in attendance at Carleton Place High School;
2. Your date of birth;
3. Reason for request (i.e. University, College, Employment, or other);
4. Contact information: email address, mailing address, phone number.
Please note that, for students who have been out of school for more than one year, a fee of $5.00, will be charged for each certified copy, payable to the school by cash or cheque. If Priority Post is required, an additional $5.00 fee will apply. There is no charge for photocopies or electronic copies.
